Responsibilities:
- Provide evaluations and therapeutic treatments in-home
- Set realistic goals with patients and monitor progress
- Educate families on exercises and recovery strategies
- Communicate regularly with care team members
What We’re Looking For:
- Licensed Physical Therapist in MN
- Strong time management and adaptability
- Reliable transportation and documentation skills
- Home health experience preferred but not required
