Travel or Local Medical Social Worker – Home Health & Hospice - Colchester, VT
We’re hiring an experienced Medical Social Worker to support Home Health and Hospice patients in beautiful Vermont. This is a 12-week contract with a firm January 12 start date , working full-time day shifts.

Requirements:
- Master’s Degree in Social Work (MSW)
- Active Vermont LMSW license
- Minimum 2 years of relevant medical social work experience
- Epic EMR experience within the past 24 months (required)
- Comfort working in patients’ homes, including winter driving conditions
- Reliable vehicle with snow tires (public transportation not available)
