Required Certifications/Degrees

Top Skills / Minimum Experience

At least 1 year of experience in social work (healthcare experience preferred)
Strong assessment, care coordination, counseling, and advocacy skills

Key Responsibilities

Complete and document psychosocial assessments to evaluate patient needs.
Develop and implement individualized care plans in collaboration with the interdisciplinary team.
Provide patient and family education, counseling, crisis intervention, and advocacy.
Coordinate referrals and community resources to support patient transitions and placements.
Participate in discharge planning and ensure safe, appropriate care continuity.
Serve as a liaison between patients, families, healthcare providers, and community agencies.
Assist with performance improvement projects, data collection, and reporting.
Prepare professional documentation such as case summaries and progress reports.
